Your alsa mixer might not be exposing the 'PCM' control. Mine, for example,
does, but it doesn't work, so I'm using 'Front' instead. Try -mixer-channel
with an appropriate value and when you're satisfied that it works, add it to
your config file, for example:
mixer-channel=Front

Of course, you can also try -ao pulse to have MPlayer use PulseAudio for sound
output.
