Well, I can't test your file, but I can say that I've never run into any
quality mishaps recently, with the exception of a long, chaotic scene on
my The Two Towers DVD that the ratecontrol didn't like. After asking on
ffmpeg-devel, I added vratetol=100000, and that fixed the problem.

Though, I haven't messed around with different lavcopts in a long time;
maybe an option you're using is messed up, or something, or I'm using
something that happens to work around a problem. What lavcopts are you
using? If you want to try mine, just for kicks, here they are.
