Ignore this patch, I just got a new sample from Carl Eugen that is not
fixed by it.

Looks like there is a second way of creating multirate files, where all
the alternative streams are packed into a single MDPR chunk (in the
"normal" multirate files you have multiple indepented audio and video
streams).

By looking at this sample, the same thing can be done also with video
streams.


I'll submit a new patch as soon as I figure this out.
