[MPlayer-dev-eng] [PATCH] Support CineForm HD

With all the other patches split out and hopefully soon committed, here are
the final changes needed (including codecs.conf entry) for supporting
CineForm HD on Linux, using the binary DirectShow codec.

I'm unsure about the registry part -- is there another, better way of doing
this? CineForm assumes these keys are set by the installer before the codec
is used.
