[MPlayer-dev-eng] [PATCH] Add DVB teletext support

Third and final patch, which actually adds DVB teletext support.
Notes:
- in libmpcodecs/dec_teletext.c (formerly stream/tvi_vbi.c) a bug has
been solved (first hunk in attached patch) which prevented
auto-detection of the character set used
- in stream/tv.h the first lines of the standard header have been
changed respect to the ones added by cehoyos a few weeks ago (rationale:
see stream/tv.c)
- teletext font autoscaling does not work unless a patch submitted by
myself back in June (at which nobody cared :-() is applied: see
http://lists.mplayerhq.hu/pipermail/mplayer-dev-eng/2009-June/061778.html.
