So,

I've done some more testing and written a small (2 file) example of the
crash.

Compile with VS = OK
Compile with cygwin = Crash
Compile with MinGW = OK

Compile

g++-4 -O0 -g3 -D_UNICODE -DUNICODE test_win.cpp paint.cpp -ld3d9
- -luser32 -o test_win.exe

So it looks like it is a problem with cygwin.

I'll report it to the cygwin mailing list
