I have tried -cache and it doesn't make a difference.

Another interesting thing to note is that with MPlayer 0.90rc5 when
using AC3 passthrough, when I hit pause, the video pauses immediately
but there is about 0.3 seconds of audio that continues to play. When I
unpause, audio resumes properly, and the next several frames of video
are sped up in order to sync up. After this everything is fine.

However, with CVS MPlayer, hitting pause immediately stops the audio
now, and so on resume there are no sped up video frames but I do get
this jittery audio problem.

So it does seem that something has changed in MPlayer since 0.90. Maybe
that information is enough to help the MPlayer gurus track it down?

Best,
Jason.
