Here's a newer version of the tvi_v4l patch, with fixes mainly the a/v 
sync code. Uncommenting REALTIME_TV changes the behavior so it's more 
suitable for realtime viewing - it shortens the capture ring buffer and 
adds a time compensation for dropped frames, which also allows pausing 
the TV. (I am currently not sure how to distinguish between mencoder and 
mplayer on the plugin level).
