Most programmers tell us that mencoder is deprecated for this kind of stuff.
You would be better off with other encoding applications like ffmpeg.

Yes. the manual has a command for producing better quality with the xvid codec.

This also depends on your DVD. to use -slang sv would  mean that "sv"
was used to denote Swedish text on the dvd.
you could use −msglevel=6 to get that complete information from the
dvd before deciding.

-vobsuboutid, -vobsuboutindex apart from telling mencoder that you
want to extact the subtitles to an idx subtitle file, will also
determine their position in the idx subtitle file.
Note that you could also have more than one subtitle in the file and
so you use the -vobsuboutid or the index to denote which one to use
when you are playing the subtitle file.

Like I said, maintenance has stopped on mencoder and it is not the
best too for this stuff anymore.
