Is this the error?: WARNING: OUTPUT FILE FORMAT IS _AVI_. See -of help.

Then use:  -of lavf

It'll automatically use the container extension.  You can also use:   
-of lavf -lavfopts format=mp4

For AAC, use this:   -fafmttag 0x706d
