This is obviously a bug. Try latest CVS, and if that doesn't work, file a full bugreport.

This is a well-known issue. Use "-noskip -mc 0"

No, it's not possible. lame doesn't understand AVIs at all. You could use some other tools to do the conversion, but you are better-off just sticking with mplayer.

What are you using to record these AVIs? If you're using mplayer, I can't imagine why you can't encode to MP3 (or anything else) in the first place.
