Try mp4 instead of m4v. It works well (out-of-the-box on idevices). I havn't found any restrictions (but I am for sure there are).

Subtitle in general are working on iPad, but I have no experiences how
to convert it into mp4. I am appreciate to hear how to do it. I have
mainly srt files.
