MP4 and M4V are different things.

Actually, there are 3 different similar containers:
- MOV (QuickTime)
- M4V (itunes)
- MP4 (ISO)

I think M4V is closer to MP4 than to MOV. In mov you can also find "edit
lists" which is not mandatory in MP4 and other minor differences. We don't
really see much mov files on the web, while these are very common in the
world where Final Cut rules.
