ffmpeg's mp4 muxer has numerous known problems which haven't been  
fixed yet. If you can use something else, mpeg4ip (http:// 
mpeg4ip.sf.net/) or (http://gpac.sourceforge.net/auth_mp4box.php)  
should work for this.
