There are (at least) three cases to consider:

1. Progressive frame pictures.
2. Interlaced frame pictures containing interleaved fields.
3. Field pictures containing only odd or even lines.

For case 2, the field order must somehow be signalled.

I've never heard of anything using more than two fields per frame, and
I hope I never will.
