[MPlayer-dev-eng] Precompiled header for mplayer

Hi,

I tried to use GCC's precompiled header for mplayer yesterday. The
time for a clean build saw merely about 7% improvement. Nonetheless I
attached the diff I used just in case that somebody may be interested
too. The dirty diff isn't intended for trunk :-)
