I have heard that the lavf MP4 de/multiplexer leaves something to be
desired with regard to AVC and they don't know how to fix it
currently. (Someone else's words, not mine.) Which leaves the best
option as -of rawvideo and multiplex using mp4box from gpac or
something similar. Or maybe some nut or matroska output in the future.

OK. What level do you suggest? After reading the man page MSGL_V seems
appropriate for the CQM correctly loaded message. But the CQM not
loaded message is an error, so should this have remained MSGL_ERR?

The punctuation will be corrected in the next patch. I will update the
patch once I obtain clarity regarding the message levels. :)
