I run multihead myself, dont have any problems there. Ok, i use xmga
instead of xv but the fs code is independent of the x11 module used.
What is your current screen layout ?
Does it crash on single screen too ?
Does it crash with -vo x11 too ?

Can't be, if you dont have mga_vid loaded, the open on the device file
will fail thus nothing can happen.

Beside you said that it's getting worse, but the x11
code wasn't touched for a damn long time, so i either
suspect that something with your x server is broken or you
use a non-aproved binary version of mplayer.
