Generally, either the PTS values in the source are off, and MPlayer is
following them to it's own peril as A/V get further out of sync. 
Alternately, the PTS values may be accurate, but so ridiculously large
that MPlayer's default (-mc) value isn't large enough to adjust so much.
 Either case can happen with files with segments of corruption / dropped
frames, as well as poorly mastered videos.

Other causes are possible, but much less common...  like a broken muxer
or audio / video encoder which allows one of the a/v streams to
progress, while stalling the other until the buffer is full.  Slow
hardware as you mention (or broken drivers), can cause this as well.
