I'm not really familiar with b-frames.  How do I add this in my case?
I've looked through all the options for lavc that refers to b-frames,
but I'm unsure.

I have figured out that the bitrate reported is the max bitrate set, not
the average.  So if I bring the average down, it works, but it's trial
and error, and re-encoding a couple of times over in some cases.
