The DVDNAV menu images are stored as a completely ordinary subtitle track.
So if you set any subtitle options that cause this track not to be player
(like -nosub or -slang with a language that does not exist for the menu)
you'll get those white blocks instead.
There are more bugs in the DVD handling code, but so far I can't be bothered
to try to fix them (it tends to be a huge effort and I personally just don't
really mind).
