try adding this to your .mplayer/config file:
af=volume=-100

I don't know if this is possible with mplayer, but smplayer (qt4 gui for
mplayer) can do this. You would just have to add '-add-to-playlist'
option in the file association menu. And also set in the
options -> preferences -> interface -> instances -> 'use only one
running instance of smplayer'
