Those are really the only two possibilities...  Seek to nearest keyframe
(fast) OR Decode the entire video (slow).

Using MPlayer either interactively or in slave mode, perhaps you could
have it seek to about 1 minute before the time you specified, then play
the video until you reach that exact time, and grab a screenshot...
