It happens when the MPlayer OSX GUI is used and video out is quartz.
In the plist at
/Applications/MPlayer\ OSX.app/Contents/Resources/External_Binaries/
mplayer_ppc.app/Contents/Info.plist
Changing LSUIElement value from 1 to 0 removes the top bar.
But it causes problems to get to the controller when video out is
MPlayer OSX
