What CPU do you use? Are there any framedrops? 

x264 codec is _very_ CPU hungry. First of all, ensure that all 
posprocesing is disabled, use xvidix vo, try -autosync with 
largevalues (e.g. 30, 50). If nothing helps, try -framedrop or 
even -hardframedrop (but that is ugly). 

Recompile mplayer from sources for your architecture from the svn 
head. Some code is heavily potimized and modern code is faster 
than two years old one.

You can also reencode movie to less CPU intesive codec, such as 
mpeg4.
