I now think I may have narrowed my problem down further.
WARNING : I have very little programming knowledge. A lot of trial and error
here !!!

I have added comments into stream/tvi_v4l.c the code at various points.
My messages are prepended with ........... in mplayers output.

I am hoping someone with more skill can see the issue, or point me in
another direction.
