you don't need to do anything special. If the button number isn't showing
in the osd there's something wrong in the the dvdnav code (quite nightmarish
if you ask me).
One thing to remember is to always run dvdnav:// with -nocache;
moreover  run these commands, please:

ls libmpdvdkit2/*.[ao]
ldd -d mplayer  |grep dvd
