The data on the DVD could be corrupt, or somehow unreadable. Or it
could be a bug in mencoder.
If you don't believe there's a problem with the DVD, you could always
try other programs such as xine, ffmpeg, transcode, (mplayer CVS) etc.,
and see if other pieces of software don't exhibit the same problem.

Well, if your DVD is dirty or scratched, then the file you dump to your
hard drive will have the same data missing, hence the same problem.
