You'r not mistaken. You'r just not reading the manual carefully enouthg.
Le't me quote "man mplayer" in the tv section:
immediatemode=<bool>
A value of 0 means capture and buffer audio
and video together (default for MEncoder).
A value of 1 (default for MPlayer) means to
do video capture only and let the audio go
through a loopback cable from the TV card
to the soundcard.
