Well, for live-streams it is often easy to explain: the rate at which data
arives at your PC is fixed, except for interruptions of course. So the
amount of data dat arrives at a certain time is the same as what needs to be
played at the same time. So it makes sence to buffer a certain amount (in
commercial products one usually sets the number of seconds instead of
cache-size but that is of course a one-to-X relation) and play untill th
ebuffer is empty. Read-ahead is impossible.

For DVD's or other media, I agree that MPlayer should try to read faster in
case the buffer runs out. However, question is if that is easy to implement
...
