hmm log below looks more like audio prob, have you tried other -ao drivers 
too? Maybe your soundcard can't handle 48kHz sound, try running mplayer with 
-srate 44100 (movie will run with 44,1kHz then (little slower but not much 
noticeable)).

Of course you should try other files first maybe you file is simply badly 
interleaved (check DOCS/* then for ways how to solve it).

alsa 0.5 can cause probs with mplayers oss out, though works fine with my 
sblive but on my laptop with ess chip I need alsa 0.9 to get well sound.
So if your playback isn't fluid (ie. image halting every few secs) try using 
alsa5 ao driver or upgrade to alsa 0.9.

yes, image isn't centered with dga and scaling, dunno why, guess it's in 
sdllib (with x11 or xv from sdl it's fine).

native dga out from mplayer is even faster, cause it doesn't use 
doublebuffering (-vo dga -bpp 15 is faster then xvideo on my system).
