Well, the patch is for encoding, not for playing video.
While you can encode videos with video-audio offset to account for
the streams not starting at the same time, sometimes it is
preferrable to have both streams start at the same time and have the
same length (as far as possible anyway, length-granularity for audio
codecs will probably make an exact match impossible most of the
time).
