Works for me.

While we're at it, I'd like an opinion on AudioStreamChangeFormat as it 
is broken on my system and do nothing except delaying start/stop about 
5s (there is a 5-retry loop with usleep 1s on failure)

Everything still works with it disabled (see attached patch).

The patch itself is a little broken since it is on top of your patch but 
diffed against trunk.
It should be easy to understand the meaning though.
And I guess we should also remove the now useless callback.
