the changes to stream_dvdnav aren't all that bad (although there's
the usual amount of nonsense that I'll have to remove), but
I'd like to hear from Uoti how  the changes to the management
of mpi fit with his patch to handle stills  and low fps video
(if they clash with each other and if Uoti's patch obsoletes
Attila's code)
