[MPlayer-dev-eng] [PATCH] remove a bunch of unused definition warnings

Hi,

here is some stuff I have sitting in my local tree.  If fixes some
unused definition warnings in a couple of different places, mostly by
adding '#if 0' around the offending code.  Maybe it would be better to
just remove the code.  I'd like to hear your opinion on this.

This touches several different files.  I have not split it up for
review, but I will split it if/when I commit.
