Well, you reported that extradata was not enough (you had to set the
resolution), but I don't know how to fix that - I just copy the extradata behind
bih and the decoder sets the correct resolution. Second thing was that it did
not show anything from Apples stream after you hardcoded the resolution, so I
guess, CoreAVC does not accept NALs that are not complete frames.

My latest patch produces complete frames and should work for CoreAVC too (if the
extradata issue can be solved, but I don't know how).
