[MPlayer-dev-eng] [PATCH] More miscellaneous -identify output

This one adds -identify output for:
- numbers of titles, chapters, and angles on a DVD;
- number of tracks on VCD and Audio CD;
- video codec of the current file;
- signal numbers;
- demuxer help header.
