committed with the following changes:
- packs are written only when totally full: reduces overhead and makes 
xvcd finally work;
typical overhead seems to be < 2% for dvd, <1.7% for [s]vcd, < 1.2% for 
generic mpeg
- initial system header updated at the second call of 
muxer_write_header, as did the original
mpeg muxer
- removed all changes in mencoder.c and the new_osh* in muxer.c
