The attached patch update the patch to apply against the latest
CVS version (where RGB32 also uses QT). FS behavior is now
what Diego suggested, i.e. each new movie is opened in
FS mode if and only if -fs was specified on the command-line.
Point 4) is no longer relevant (the RGB32-in-QT fixed that
already, it seems).

The patch also remove a memory leak in RGB32 mode, static-ify
all globals in vo_quartz, fixes going in FS mode on the
first hit of the 'f' (or whatever) key.
