[MPlayer-dev-eng] [PATCH] Enable key repeats with vo_quartz

This small patch makes vo_quartz listen for key repeats, if you keep a
key pressed, it'll keep doing what it just did. An example: You no
longer have to push the key several times to decrease the volume, simply
keeping the key pressed does the job.

As part of this patch, I took the liberty of separating the huge switch
statement in the Carbon callback into a separate function.
