[MPlayer-dev-eng] latest mingw network patch

hi,

this is as promised the latest version of the patch to make the
network stuff work on windows with mingw.

i moved the winsock init/uninit stuff to stream.c, which is maybe
ugly, but the easiest and safest way (i made a note (to myself) to
move it to a better place later though, most important thing is that
it works for now). i hope the configure stuff is clean enough that
way too.

hopefully i didnt miss/overlook anything, this patch works well for
me. you might consider enabling cache2 on mingw by default, since
sascha sommer ported it to windows the other day :) it really
improves the experience.

PS: this needs the latest version of sys/types.h from mingw cvs to
work, because otherwise ssize_t isnt defined.
