I couldn't get quicktime audio to work until I copied QuickTime.qts to 
c:/windows/system.

handler = LoadLibraryA("QuickTime.qts"); (loader/qtx/qtxload.c)

was failing on winxp / mingw because that file was only in 
/usr/local/lib/win32 and not in c:/windows/system.

The attached patch adds some tips for mingw users.
