Sorry this is not a reply, I accidently deleted original letter.

I know about two fixes unapplied:
vo_sdl 'ESC' bug - pressing ESC always behaves as if 'q' was pressed. In
cvs it correctly generates 'ESC' keypress (which quits by default
anyway ;), but one can redefine it)

menu + pause bug - mplayer with menu=on eats each first pause keypress
before showing menu (and releases it afterwards). In
mplayer-menu_bug.patch some lines from vf_menu.c are commented out,
which fixes this and now there is no reason not to turn on menu by
default in config.
