You can use track numbers with dvdnav.  Just change dvd: to dvdnav:
dvdnav://1


I have seen times where the time stamps were off for some reason.  I'm
not on a machine with it handy, I think there are a couple of flags
for changing how to get timestamps and that might help.    In the end,
idx files are plain text., occasional manual edits might work.
