Well, I encode a lot to H.264 using the AVI container with MEncoder and so far 
my encodes can be played with MPlayer, VLC and Xine. I always use B-Frames 
when encoding to H.264 and didn't had any problems with my encodes.

I cannot comment on Windows players as I don't have it here :)
