Mencoder's MP4 muxer (via lavf) works fine, and is accepted by iTunes. 
Has been for at least a year.

~750 is the maximum bitrate for 320:240 content, but 640:480 content
(with the proper hacks for iTunes) can use ~1.5mbps.
