Matt,  

The best results I've seen were from the command lines
below.  This method was taken from section 7.11 of the
MPlayer-1.0pre5 manual titled "Making a high quality
MPEG4 ("DivX") rip of a DVD movie"
DOCS/HTML/en/menc-feat-dvd-mpeg4.html).  And it does
what the author purports - the quality of the result
is simply breath taking.

Note that this is a 2 pass procedure (vpass is the
only option that changes) and you need to run mencoder
twice.

mencoder dvd://1 -oac mp3lame -vf
crop=720:352:0:62,hqdn3d=2:1:2 -ovc lavc -lavcopts
vcodec=mpeg4:vbitrate=2400:v4mv:mbd=2:trell:cmp=3:subcmp=3:mbcmp=3:autoaspect:vpass=2
-o tmp/01-U2-Go\ Home\ DVD.hq-Elevation.avi

mencoder dvd://1 -oac mp3lame -vf
