[Libav-user] Frames are not visually lining up between my code and ffmpeg.exe

I've confirmed this does seem to be a regression.

I ran the same code with the same video on ffmpeg 0.8.3 and 0.11.1 and you
can see the thumbnails generated are not the same. #281 in 0.11.1 lines up
with #221 in 0.8.3.
