Usually, but not always. Anyhow, I like using ffmpeg for many batch operations before starting an edit, so it's important to minimize generational loss from re-encoding.

Would it be possible to create a bitstream filter to do this?

Perhaps. But it obviously doesn't have the same scripting options, nor the wide format support of ffmpeg. I wonder if anyone else is interested in this feature.
