Sorry if I was unclear before:
If you want this fixed, you need to implement the 
specification linked in this email thread. There 
are many problems with FFmpeg that are not 
completely understood and where trying different 
things may help finding a solution. The problem 
you have is completely (100%) understood and a 
specification is (freely) available.
I cannot work on it because I wouldn't know how 
to test and I suspect this is true for (all?) 
other developers...
